Referral Process
We try to make our referral process as easy as possible for everyone. We can accept a referral over the phone, email, or through fax. We do not require a doctor’s referral, but accept referrals from anyone, whether it be a family member, caregiver, professional, medical staff, self-referral, or other. Our goal is to help as many people as possible, and this helps us reach this goal.
To make a referral for yourself or someone else through fax, fill out the referral form below and fax it and copies of insurance cards to 919-300-7943. Our fax is confidential and HIPAA compliant, but please call to check the fax was received.
To make a referral for yourself or someone else through email, fill out the referral form below and send it and copies of insurance cards to your Marketing Relationship Specialist. Our email is confidential and HIPAA compliant.

To make a referral for yourself or someone else over the phone, please call the office or your Marketing Relationship Specialist, and we would be happy to complete a referral for you. You may also pass the contact information for the potential client to your Marketing Relationship Specialist and they will personally reach out if that works better.

Once we receive a complete referral, we will reach out to the client to answer any questions they may have. We will then check their insurance and call the client to confirm they accept any charges that are not covered by insurance. If they accept, we pass the contact information to their therapist, who will reach out directly to the client to schedule their first appointment.
Insurance
We are Medicare Part B approved providers! We also accept private insurances, self-pay, and can offer a sliding scale rate on a case by case basis. However, we are unable to accept Medicaid or Medicare Part A. If you are unsure about your insurance coverage, or wish for clarification, please give us a call, and we are happy to look into it for you!
